Refills may be requested by calling our main number at (502) 863-6426. Please have the following informaton ready:

• The patient's name and phone number
• The name and phone number of the pharmacy
• The prescription number

Please give the office 48 hours notice of Ritalin, Adderall, Dexedrine, and other controlled drugs that require written prescription.
