Kentucky Providers Recognized for Healthcare Transformation Efforts at 7th Annual Kentucky eHealth Summit

Kentucky Regional Extension Center, based at the University of Kentucky, honored eight healthcare provider organizations for leading the charge to transform primary care in Kentucky.

The “Practice Transformation Award” was presented to the following members of Kentucky REC’s inaugural Patient-Centered Medical Home (PCMH) cohort in recognition of their commitment to improving healthcare quality outcomes and cost:

Bluegrass Community Family Practice – Bardstown, KY
Central Internal Medicine – Lexington, KY
Drs. Borders and Associates – Lexington, KY
Family Medicine Clinic of Danville – Danville, KY
Georgetown Pediatrics – Georgetown, KY
Primary Care Centers of Eastern Kentucky – Hazard, KY
UK Family and Community Medicine – Lexington, KY
UK HealthCare – Georgetown – Georgetown, KY

These practices are working to achieve National Committee for Quality Assurance (NCQA) PCMH Recognition. A Patient-Centered Medical Home is a team-based model of care led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes. The PCMH practice is responsible for providing for all of a patient's health care needs or appropriately arranging care with other qualified professionals. This includes the provision of preventive services, treatment of acute and chronic illness, and assistance with end-of-life issues. The team members work collaboratively to provide high levels of care, access, communication, care coordination, integration and safety.

Medical homes can lead to higher quality and lower costs, and can improve experience of care for both the patient and provider.

Recipients of the award were recognized on Tuesday, September 16 at the 7th Annual Kentucky eHealth Summit in Louisville, which brings together parties invested in advancing Health Information Technology in Kentucky. Sponsored by the Governor’s Office of Electronic Health Information, the event draws hundreds from around the state who come to present ideas and listen to state and national leaders speak about new initiatives and opportunities in Health Information Technology.
